Medical Assistance
Medical Assistance under the Fund covers MRIs, CT Scans, Blood Tests, Ultra Sounds, Prescriptions, Major Surgeries, Physiotherapy and Maternity Benefit.
Requirements for MRIs, CT Scans, Blood Tests & Ultra Sounds
1. Application form (See below)
2. Most recent pay slip or Employment Number
3. Marriage Certificate in the case of a spouse
4. Letter from Justice in the case of a Common Law Relationship (the relationship MUST be five years old and more)
5. Birth Certificate for Child reflecting names of parents – Where the father is the applicant and there is no reflection of his name on the Birth Certificate, then a letter from Justice is needed for verification of relationship)
6. Request Form from the Doctor
7. Costing from the Vendor
8. Letter of undertaking from health insurance provider.

3. Marriage Certificate in the case of a spouse
4. Letter from Justice in the case of a Common Law Relationship (the relationship MUST be five years old and more)
5. Birth Certificate for Child reflecting name of parents- Where the father is the applicant and there is no reflection of his name on the Birth Certificate, then a letter from Justice is needed for verification of relationship)
6. Original Prescription
7. Provision is made on the application form for the member’s health insurance policy number. If the health insurance is exhausted then assistance is calculated using the respective percentage rates.

3. Marriage Certificate in the case of a spouse
4. Letter from Justice in the case of a Common Law Relationship (the relationship MUST be five years old and more)
5. Birth Certificate for Child reflecting names of parent - Where the father is the applicant and there is no reflection of his name on the Birth Certificate, then a letter from Justice is needed for verification of relationship)
6. Diagnosis Letter from Doctor which also should reflect the cost
7. Letter of undertaking from health insurance provider

4. Letter from Justice in the case of a Common Law Relationship (the relationship MUST be five years old and more)
5. Birth Certificate for Child reflecting names of parents -Where the father is the applicant and there is no reflection of his name on the Birth Certificate, then a letter from Justice is needed for verification of relationship)
6. Letter from Physiotherapist reflecting cost – Where the RPT can state distinctly the length of treatment then it MUST form part of the document.
Requirements for Maternity Benefit
1. Written application
2. Most recent pay slip or Employment Number
3. Marriage Certificate in the case of a spouse
4. Letter from Justice in the case of a Common Law Relationship (the relationship MUST be five years old and more)
5. Birth Certificate for Child reflecting names of parents -Where the father is the applicant and there is no reflection of his name on the Birth Certificate, then a letter from Justice is needed for verification of relationship)
6. Letter from Doctor to say what sort of delivery to be done – Normal or C-Section Delivery. In the case of a C-Section, the Dr. MUST put in writing what complications there are which merit such.
7. Letter from health insurance provider stating what portion of MONEY was paid or is to be paid to whom
8. If there is a need for reimbursement, then all of the above along with the original receipts MUST be presented.
N.B. There is a ninety day period that is observed on all applications. Once an application is not made within that period, the decision is left to the discretion of the Management Committee of the Fund.
